Service Dog Application Process:
**Please Note: you must be willing to commit to attending training for up to 18 months if applying for a puppy, or the "Puppy Raise to Completion" Program. Thank you.**
Contact us via mail, phone, or e-mail; we will conduct an informal, preliminary interview with you, and answer any questions you have.
Submit an application and a $50, non-refundable application fee to MWAM (you can submit this fee mailing it to Mutts with A Mission, P.O. Box 4147, Suffolk, VA 23439). You can download the application from our site or call to have an application mailed to you.
Completed Medical History forms are returned to MWAM.
MWAM will review your application and call you with any questions we may have regarding your application.
MWAM will contact your references.
MWAM will contact you for an interview that includes everyone who lives in your household.
You demonstrate the ability to pay for the daily expenses of caring for a dog, including, but not limited to, food, vet care, heartworm medication, and replacement equipment.
If providing your own dog, we will conduct a temperament test on your dog to determine suitability for service dog work. Not all dogs will be found suitable..
We perform a final review following your interview. You will not receive acceptance/rejection notification on the same day of your interview. If you do not have a dog or your dog was not suitable for training we will discuss your needs and possible placement of a dog to train through our program.
We aim for “perfect” in everything we do. The same is true when we match of a dog with a client. When a suitable dog has been identified, we will contact you to arrange meeting the dog, signing the contract, and schedule training.
If MWAM finds a dog for the applicant the dog remains the property of MWAM through out training. Ownership is granted after one year and successful passing of the re-certification test.
