Owner-Trained Service Dog Applications
Do you have a dog you think may be a candidate for owner-training?
Please review our qualifications and fill out the questionnaire below to receive an application.
Application Process:
Submit the pre-questionnaire.
Completed application. The portions completed by your veterinarian and medical doctor, psychiatrist, psychologist, or licensed mental health professional must be current. Forms dated more than 30 days prior to the date the application is submitted will not be considered.

If active duty, you must provide written authorization from your command. You will need to research your branch’s policies related to Service Dogs for Active Duty Members. If your branch does not allow Active Duty members to utilize Service Dogs you will need to have an ETS or Med Board date.
Submit all required information.
Submit application fee of $300.
Packet will be reviewed for completeness.
Interview will be scheduled.
If accepted the dog will be tested for suitability.
If dog passes suitability test training will be scheduled.
Minimum Qualifications:
Honorable Discharge or current honorable service.
Verifiable PTSD, MST, TBI, or mobility disability.
Disability MUST have been incurred during your time of service and be service-connected.
No convictions of any crimes against animals or types of abuse.
Stable living environment.
Free of substance abuse. Using Marijuana, smoking, and vaping is not allowed in the dog’s environment.
For all psychiatric disabilities, individuals must be actively participating in mental health treatment. Medication appointments are not included.
Dog must be 2 years old or younger.
Dog must be healthy and physically sound.
Dog must be neutered/spayed prior to graduation (there are NO exceptions to this rule).
Certain breeds will not be considered for owner-training (please visit our FAQ page).
Incomplete applications will not be reviewed.
Please note we DO NOT place Service or Facility Dogs in homes with individuals who smoke or vape for the health of our dogs.
Not only are you committing to a relationship with the dog, but you are also committing to a relationship with Mutts With A Mission for the working life of the dog. Once you receive a Mutts With A Mission dog, you are required to stay engaged with MWAM, provide veterinary updates as requested, and meet with MWAM staff in person at least once every year (including recertifications) or more if required.
